These are only available from a licensed medical examiner, as listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. Once you receive one they are generally good for two years, unless you have a condition that needs monitoring. Then, you’re also required to show proof of permanent residency or citizenship. For this a social security card or birth certificate is the kind of thing that is usually necessary. Lastly you just need a valid driver’s license.
