The October 2024 rule change
In October 2024, the California DMV eliminated the mandatory written knowledge test for drivers 70 and older who have a clean driving record. Drivers in this age group who have no at-fault accidents, no failures to appear, and no recent license suspensions can now renew online or by mail without retesting. Drivers with a flagged record still take the written test.
This was a real shift. California had previously required 70+ drivers to retake the written test at every in-person renewal - one of the stricter regimes in the country. The change narrows testing to the drivers whose records actually suggest a need for review, rather than triggering it by age alone. It also brings California closer to states like Oregon and Washington that use medical and record-based triggers instead of universal age triggers.
The vision test at 70 and older remains in place.
Renewal cycle and documentation
California has a five-year renewal cycle for all ages. Drivers 70 and older cannot use online renewal past 80 in most cases; California blocks online renewal at 80 entirely and restricts it with conditions between 70 and 79.
California renewals stay simple if you have a clean record. The vision check is the one element that doesn't go away.
What you bring to a renewal:
- Current license
- Proof of legal presence (REAL ID)
- Vision screening (at the office or a completed DL 62 from a licensed eye specialist within 90 days)
- Fee (currently $45; waived for Senior Citizen ID)
- Written or road test only if triggered by record or referral
Mandatory physician reporting for dementia
California is one of only five states where physicians must report certain conditions to the DMV. California's rule is specifically about dementia and related disorders (Alzheimer's disease, Lewy body dementia, frontotemporal dementia). Physicians are required to submit Form DL 329 to the California Department of Public Health, which then forwards the report to the DMV.
The rule is narrower than Pennsylvania's (which covers any condition impairing safe driving) or New Jersey's (seizures, unconsciousness, motor coordination). California went after the specific conditions most correlated with at-fault crashes in older drivers: cognitive decline that affects judgment, wayfinding, and reaction time.
If you are a California family member concerned about a parent's diagnosis, the physician is a powerful ally: a Form DL 329 submission carries more weight than a family report and triggers a structured review rather than an informal inquiry.
License restrictions - one of the broadest menus in the US
California has one of the most extensive menus of graduated restrictions in the country. Many older Californians voluntarily accept one or two of these to extend their driving years instead of full surrender.
Common senior-driver restrictions:
- Corrective lenses required (vision-based, any age)
- Daytime driving only
- No freeway (local roads only)
- Speed-limit restrictions (no roads over 45 mph, for example)
- Geographic radius (a specific distance from home or within named counties)
- No rush-hour driving (typically 7-9 a.m. and 4-6 p.m.)
- Adaptive equipment required (right-side mirror, steering knob)
- No passengers under 21
Restrictions are assigned after a Driver Safety reexamination or at the driver's request during renewal. Many older Californians voluntarily accept one or two restrictions to extend their driving years - a strategy that is underpromoted by the DMV but worth asking about before accepting full license suspension.
Senior Citizen ID card
California issues a free, non-expiring Senior Citizen ID card to residents 62 and older. The card serves as primary legal identification for banking, voting, boarding domestic flights (REAL ID), age verification at healthcare providers, and any other situation that previously required a driver's license. Apply at any DMV field office with proof of age and residency.
For a senior considering voluntary license surrender, the Senior Citizen ID is the first thing to arrange. Many older Californians delay surrender because they do not know there is a free, dignified alternative to carrying an expired license.
Spanish-language resources
California DMV publishes its driver's handbook and many renewal forms in Spanish. The senior-driver-specific DL 329 and Senior Citizen ID application are both available in Spanish. Our California pages include Spanish-language practice tests (Phase 2 expansion).
Medical Advisory Board and reexamination
The California DMV Driver Safety branch uses a Medical Advisory Board to review complex cases. A referral can come from a physician (Form DL 329), a family member, a law enforcement officer, or the DMV itself during a renewal flagged for review.
Possible Board outcomes:
- Full license reinstatement
- Restrictions
- Reexamination (written, vision, or road)
- Probationary license with periodic medical reports
- Suspension or revocation
The process is administrative, not criminal. Drivers have the right to a reexamination hearing and may be represented by counsel.
